MEMBERSHIP PRIMER

• Membership Requirements

  The applicant should not be more than 54 years old at the date of membership.

  Required Documents:

  1. Submit a photocopy of the following documents and present the original documents for verification.

  • Appointment Papers;

  • Updated Service Record;

  • Board Rating or G.S.I.S. Policy;

  • Must have at least Four Thousand Pesos(Php 4,000.00) salary NET take home pay for their     latest payroll;

  • Latest (3)Three consecutive months of payslip.

  2. Present at least two (2) valid photo-bearing ID’s that contains name, signature and residential address;

  3. Submit two (2) pieces of 1” x 1” ID picture;

  4. Submit loan application form that is properly and completely filled up by the applicant and four (4) co-makers who are active members of the Association and belongs to the same division and station;

  5. Loan application form must also be signed by the Principal or Officer in Charge citing that the borrower and the co-makers are not presently on leave nor they have any intention to resign or retire in the next one (1) year.

  Procedure for Membership:

  1. An applicant should personally appear and submit all the requirements pertaining to his/her membership for screening.

  2. Upon completion and submission of all requirements, the applicant should properly fill-up the Application for Membership Form and Signature Cards. All these documents shall be forwarded to the Manager or to the Approving Authority duly delegated by the Board of Trustees for approval.

  3. Upon approval of the Membership Application, the applicant shall be required to pay the amount of Two Hundred Sixty Pesos (Php 260.00) for:

  **This would entitle a member for a death benefit of Php35,000.00

  Correspondingly, the Association shall issue an ID, membership certificate, and a passbook.
